a)  I hereby declare that;-

1. The information given is true to the best of my knowledge and belief & I have not been debarred from appearing at any examination held by any Government Constituted or statutory examination authority of India.
2. I am aware that I am taking admission to a autonomous professional training course.

iii)       If admitted, I will be governed by the rules and regulation, in force and those may be made applicable hereafter by the academy.
iv)        As long as I am a student of the academy, I will not involve myself in any kind of activity, inside or outside the academy, I will not harm the image of the academy institute in the society. I shall not participate in any unsocial activity; otherwise I am ready to accept any disciplinary action taken by the academy authority, such as rustication or detention.
